Individual Damage Assessment Forms 

Click HERE to complete the on-line individual damage assessment form. Paper copies are available at Village Hall. 

Village staff will be available to answer questions if necessary. Please call Village Hall at 708-345-0020. The form may be submitted online or dropped off at the Westchester Village Hall.


Salvation Army Flood Clean-Up Kits Available 
Residents of Proviso Township are advised that Salvation Army Flood Clean-up Kits are available to be picked up at the Westchester Village Hall, located at 10300 W. Roosevelt Rd., Westchester, IL 60154, Monday through Friday from 7:30 am to 3:30 pm. The kits may be obtained in Public Works, on the West side of the building. Please be aware, there is a limited quantity of kits available for distribution. They will be distributed on a first come, first served basis.
